The U.S. Embassy in Santo Domingo is pleased to announce that Representatives from the Regional Federal Benefits Unit will visit Santiago to offer services for beneficiaries or individuals with questions about U.S. Social Security and other federal benefits.

We remind you that beneficiaries residing in Dominican Republic must complete their annual report to the Social Security Administration and you can complete and submit the form during the service hours offered in Santiago.

General Town Hall Meeting:

The Regional Federal Benefits Unit (FBU) of the American Embassy in Santo Domingo is responsible for providing services to all the Caribbean, Central and South America countries. We assist U.S. citizens and foreign nationals who have inquiries or receive benefits from the Social Security Administration, Railroad Retirement Board, Office of Personnel Management, and the Department of Veterans Affairs. We have staff located in three U.S. Embassies: Santo Domingo, San Jose, and Buenos Aires. Individual consultations – Federal Benefits Unit

The representatives from the Regional Federal Benefits Unit will provide individual consultations to beneficiaries and those with inquiries related to U.S. Social Security and other federal benefits on Wednesday November 20th.

Schedule for consultations will be held from 9am to 2pm exclusively in Classroom A22 at Centro Cultural Dominico Americano.

NO appointment necessary.-

First come, first serve assistance to be provided.

Please bring your original documents (US Passport, Cédula, etc) and a photocopy if you are submitting any kind of application.

WHAT TO BRING TO YOUR INDIVIDUAL CONSULTATION APPOINTMENT FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):

 

Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth Certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates

SSA Proof of Life Study: In 2019, SSA mailed the questionnaire to beneficiaries receiving their own benefits in Dominican Republic. Please bring copy of your passport and a completed SSA- 7162

Social Security Number Card Application: Bring original and a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.

Change of Address for Social Security: Bring your current passport.

Medicare Part B Enrollment/Cancellation: To enroll in Medicare, you should complete and sign this CMS-40B and to cancel your enrollment please complete and sign this CMS-2690
