Passport renewals for Minors (under age 18)

The minor applicant must appear in person, and in most cases, both parents must be present. Please bring the following documents with you.

  • Certified birth certificate (showing parents’ names) and a copy
  • Form DS-11 completed ONLINE and printed. Do not sign the application.
  • Minor’s most recent U.S. passport and a photocopy of the bio/photo page
  • Application fee: $135.00 or equivalent in Dominican pesos.
  • Two 2″ by 2″ (5 cm x 5 cm) color photos (front view, full face and plain white or off-white background.
  • Photo identification of both parents (present the originals and submit one copy of each parent per applying child)
  • If one parent is unable to attend
    • Applicant is under age 16, the absent parent must notarize a DS 3053 with a US Notary. Original notarized DS 3053 and copy of ID used are required.
    • Applicant is age 16 or 17, only one parent is required to consent either through physical presence or notarized DS 3053.
  • Progression photos of the minor over the years since the issuance of the previous passport and up to current date.
