Passport Renewals for Adults

Renew by MAIL (DOMEX) with a DS82 if your passport is:

  • Undamaged;
  • Issued when you were 16 years of age or older;
  • Issued within the last 15 years;
  • Issued with a 10 year validity; and

Issued in your current name OR you have changed your name since your passport was issued and can submit legal documentation to prove this change

Send the following items

  • Your most recent U.S. passport and a copy ·
  • Two 2″ by 2″ (5 cm x 5 cm) color photos (front view, full face and plain white or off-white background.
  • Form DS-82 completed online, printed, and signed.
  • Fee paid through pay.gov
  • If applicable: Evidence of Change of Name – e.g. Marriage Certificate, Statutory Declaration, or Court Order (name change must have taken place after your passport was issued)

If any of the conditions above do not apply, make an appointment and bring the following documents:

  • Proof of citizenship and a copy: U.S. passport; US Birth Certificate or CRBA; or Naturalization Certificate
  • A Valid ID: cedula, driver’s license, passport
  • Form DS-11 completed online and printed. Do not sign the application.
  • Two 2″ by 2″ (5 cm x 5 cm) color photos (front view, full face and plain white or off-white background.
  • Application fee: up to $165.00 or equivalent in Dominican pesos.
