If your valid U.S. visa has been lost or stolen, immediately notify the U.S. Embassy Santo Domingo Consular Section. In order to submit your report, please email support-dominicanrepublic@ustraveldocs.com including following documents:
- Report from the National Police indicating that your passport was lost or stolen; and
- Letter of Explanation including the date, circumstances, and location of the visa’s loss or theft. Make sure you include all your biographic information including your complete name, cedula number, date of birth, and place of birth.
Applying for a New Visa
If your visa was lost or stolen and you want to apply for a new visa, please follow the information provided in the Make an Appointment section. You will not qualify for a renewal appointment since you no longer have your visa in your possession. You must bring all the required documents (listed above) with you to the interview. There is no guarantee that that you will be entitled to another visa. Please remember that it is your responsibility to safeguard your visa against loss or theft.
