When filling out your DS-260 form for an immigrant visa, the information must appear as in the passport. Said form must be completely and correctly filled.

When consular officers review your visa application, they use the information on your form to process the case. For this reason, it is important that your DS-260 is complete with the correct information. Your passport information must be the same as in the form.

For example, if your correct name on your passport is Juan José Ramírez Santos, you must write it in full on your form: Juan José Ramírez Santos. Including all your first names and all your last names, exactly as in your passport.
