Complete your Form DS-260 as it is in your Passport

When filling out your DS-260 form for an immigrant visa, the information you enter on the form must match the information in your passport.  Make sure you thoroughly and correctly complete the form.

When consular officers review your visa application, they use the information on your DS-260 form to process your case.  For this reason, your DS-260 must be complete, correct, and match the information in your passport.

For example, you should include all your first names and all your last names, exactly as they are in your passport.  If your correct name in your passport is Juan José Ramírez Santos, you must write it in full on your DS-260 form, i.e., Juan José Ramírez Santos.
