Come prepared to your notarial appointment in the Embassy

The American Citizen Services section of the U.S. Embassy in Santo Domingo would like to remind you that if you bring documents to be notarized that require a witness or witnesses, it is your responsibility to bring them to the appointment. In addition, the witnesses must be of legal age and provide a government issued photo ID (for example cedula or passport). Please note that Consular officers or Embassy staff cannot serve as witnesses for these purposes.  

 Consent for the issuance of a passport to minors (Form DS-3053) does not require witnesses.
